SUPERVISORS' TITLE: Medical Director (clinically) and Program Director (administratively)
AVAILABLE TIMES TO SERVICE CLIENTS : Monday through Friday, 8:00 a.m. to 5:00 p.m., with 1-hour break from 12:00 p.m. to 1:00 p.m. each day., Occasional weekends, afterhours and holidays.
SUMMARY OF POSITION RESPONSIBILITIES :
Community Wellness Center is a community mental health service provider that tailors specialized therapeutic and somatic treatment services to emotionally and behaviorally challenged youth, adults, and families. Our Outpatient Mental Health Clinic provides community-based services that improve the quality of life for individuals with mental health challenges and social impairments. Our strengths-based rehabilitation services are designed to produce healing and restoration in communities.
The Certified Registered Nurse Practitioner shall provide general and preventative care, conduct check-ups, treat illnesses, order lab tests and prescribe medication for children and adults who are participants in the program. CRNP is expected to service clients in a multi-disciplinary collaborative approach. For many patients you will serve as the primary care provider.
ESSENTIAL DUTIES :
- Participate in staff collaboration meetings regularly for coordination of care
- Manage staff at the clinic location of assignment
- Service at least 10 patients per day
- Provision of leadership to the treatment team
- Update and develop Individual Treatment Plans as needed
* Assists Medical Director in developing clinical practice guidelines.
- Participates in provider and medical care team meetings.
- Establishes positive relationships with local medical community and attends local medical society functions.
- Interviews patients to obtain history, performs physical examinations, orders lab and other tests, prescribes medications and treatments.
- Provides continuity in managed care for patients with pre-existing long term problems.
- Makes referrals for secondary and tertiary care.
- Performs medical procedures according to privileges issued.
- Provides health maintenance visits, evaluates for immunizations and provides anticipatory guidance and referrals; performs contract and special physical examinations as EPSDT, Head Start, employment, etc.; performs STD (sexually transmitted disease) and family planning screening and education.
- Documents and codes all patient contact accurately and legibly in the medical record.
- Performs as an active member of the medical team.
- Performs other duties as assigned.
- Participate in in-service instruction at least every 6-months.
- Other duties as assigned, including but not limited to participation on clinical committees (i.e. Infection Control Committee, Environment of Care Committee), providing education, Quality Review and/or other activities.
- Develop a system of nursing by executing and introducing substitute solutions that will handle intricate system problems and/or patient care situations

- Function as a clinical professional in the transfer of perioperative patient care
- Offer assistance by providing surgical care in relation with state, federal and facility requirements
- Combine research into the perioperative clinical set up through execution of evidence based practice
- Establish, execute, organize, and assess educational programs for the community, perioperative patients, their families and the staff
- Act as an agent of transformation to establish performance standard systems and to improve perioperative patient results
- Encourage patient care by working together with nursing staff in areas such as evaluating current practices, reviewing alternatives, deliberating with patient care administrators and offering assessment to personnel
- Take decisive steps on allocation of resources and personnel
- Generate special treatment plans following after patient examinations
- Give education to patients and their families on managing their situations.
- Integrate practices to enhance staff synergy.
POSITIONS SUPERVISED: Registered medical assistants, administrator, receptionist, registered nurse case managers, licensed practical nurses
LEVEL OF EDUCATION / TRAINING / QUALIFICATIONS :
- Master’s degree in clinical nurse specialization with focus from an authorized school of nursing
- Minimum two (2+) plus years of experience providing direct support to adults with severe persistent mental illness and substance abuse disorders. These will be verified through your professional references provided.
- 45 -55 wpm typing; must demonstrate typing skills.
- Ability to apply the concepts of customer service in the workplace. Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.
- Posses a basic knowledge of all office equipment, including word processors, typewriters, Dictaphone, FAX, and copy machines, and a willingness to attend appropriate classes to increase knowledge as needed.
- Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills
- Ability to handle multiple tasks and meet critical deadlines. Ability to meet short and long-term goals and project deadlines.
- Respect for the confidentiality of all communications required within job duties, including client/employee records and documents, and electronic transmission by voice, data, and wire
- Must demonstrate you are free from TB, measles, mumps, rubella and varicella
- Must pass a CJIS criminal background check and have no criminal convictions or history that indicate behavior which pose a potential threat to clients and staff
- Must have a driver's license and your own transportation

- Outstanding ability to care for others and take up leadership functions in areas related to healthcare.
- Must be 18 years or older
- Current CPR and first aid certifications

- Ability to follow CWC Clinic practice guidelines – including adhering to the CWC immunization policy and the CWC Mission and Vision and CWC Corporate Compliance Policy.
- Ability to receive constructive feedback including evaluation of provider’s productivity and practice attributes.
REQUIRED CERTIFICATION / LICENSE:
- Certified Registered Nurse Practitioner with the Maryland Board of Nursing is required
- Registered with the Maryland Board of Nursing as a delegating registered nurse is preferred and may be later required
- Certified with a professional nurse practitioner board is preferred and may be later required
- DEA certification is required
- CDS license is required
- Registration with the Maryland Medical Cannabis Commission is preferred and may be later required
- Approval as a Suboxone / Naxalone prescriber is preferred and may be later required
- Experience with EMR ICANotes or HealthFusion is preferred
PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.
WORKING CONDITIONS: Work is performed in an clinic setting near downtown Baltimore City; Work exposes incumbent to contagious or infectious diseases; Work may expose incumbent to dangerous and volatile situations; which could result in bodily injury.
ANTICIPATED TRAVEL: 5% - 10% of the time
